Thank you for your interest in becoming a donor with F. Williams Egg Donor Services. Please review our For Donors page for complete information on becoming a donor.
Our On-line application process is as follows:
1.) Complete and submit our application on-line, or by fax, e-mail attachment or post mail.
2.) E-mail recent, color photos (a close up of your face as well as a full body shot).
3.) We review your application and determine if you qualify to become a potential donor.
4.) We contact you by e-mail or phone and send you our complete donor packet via e-mail.
5.) We answer any and all your questions and concerns to make sure you completely understand the process.
5.) Your photos and profile are placed on our on-line donor database for intended parents to review.
After reviewing your online application we will contact you within 48 hours if you qualify for our program.
You must be height/weight proportionate, and not excessively overweight. Please see the weight chart below for an idea of the proportions. The numbers are not exact, so you can be somewhat more, but not more than 10 lbs. The proportions are required for the health of the donor.
The information you provide is confidential, however prospective intended parents will review the information, along with your photos.
After you submit the online application, please email your photos to photos@fwdonoreggs.com. or you can upload them using the link below. Please include your name along with the photos.
Most of our correspondence is done by way of e-mail, so please provide an e-mail you check often. Also,please remember to include the photos with your application. We cannot review any application without photos. So please do not forget to send your photos.
And Most of all, please only apply if you are "certain" and "serious" about becoming a donor.
